Appointments and financial information

You can book an appointment by phone or through the online booking form on our appointment and contact pages, which is provided by PatientXpress. Follow the confirmation it gives you, and call us if the status of an appointment is unclear. Availability may change. Applicable appointment and payment conditions will be provided separately; these terms introduce no cancellation fee or financial agreement.

Booking an appointment is not consent to treatment. Treatment is discussed and consent is obtained at the practice. For a minor, a parent or legal guardian generally gives consent, except where California law allows the minor to consent.

Insurance and financing information is general. Coverage, benefits and financing eligibility depend on the relevant plan or provider and your circumstances. The practice can follow up with your insurer and submit claims, but this does not guarantee payment. A name or logo on the website does not guarantee payment, eligibility or a particular network status.

Content and appropriate use

Content is owned by the practice or used with permission, unless otherwise indicated. Personal informational use and uses permitted by law are allowed.
